I had some fun with forms a while ago. A neat way to build back­ground info (and para­noia) for your Sole Sur­vivor char­ac­ter! A few peo­ple asked me if I had them, and truth­ful­ly, I built the them all in Pho­to­shop, which I'm not sure you're aware, but is NOT a word proces­sor. Who knew?

But I con­vert­ed them to PDF for your use. Unfor­tu­nate­ly, I don't have the nec­es­sary soft­ware to make them actu­al forms, but feel free to use and dis­trib­ute to your friends. You don't even need to cred­it me. It was just a fun lit­tle exer­cise to get inside the head of my character. 🙂
